About the Role:
As a Physical Therapist at Union County Nursing Home (UCNH), you will play a crucial role in enhancing the quality of life for our residents through effective rehabilitation strategies. Your primary focus will be on assessing patients' physical conditions and developing personalized treatment plans that address their unique needs, particularly in orthopedic and geriatric rehabilitation. You will collaborate with a multidisciplinary team to ensure comprehensive care, utilizing manual therapy techniques and evidence-based practices to promote recovery and mobility. Additionally, you will educate patients and their families about injury prevention and wellness strategies, empowering them to take an active role in their rehabilitation journey. Ultimately, your expertise will contribute significantly to the overall health and well-being of our residents, fostering a supportive and healing environment.
Minimum Qualifications:
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Current state licensure or eligibility for licensure as a Physical Therapist.
- Experience in orthopedic and geriatric rehabilitation settings.
Preferred Qualifications:
- Certification in specialized areas of physical therapy.
- Experience working in a long-term care or nursing home environment.
- Bilingual skills to effectively communicate with a diverse patient population.
Responsibilities:
- Conduct thorough assessments of patients' physical abilities and limitations to develop individualized treatment plans.
- Implement and monitor rehabilitation programs, adjusting techniques as necessary to optimize patient outcomes.
- Provide manual therapy and therapeutic exercises to improve strength, flexibility, and mobility.
- Educate patients and their families on rehabilitation processes, injury prevention, and wellness strategies.
- Collaborate with healthcare professionals to ensure a holistic approach to patient care and progress tracking.
Skills:
The required skills in physical therapy and rehabilitation are essential for assessing and treating patients effectively on a daily basis. You will utilize your knowledge of orthopedic and neurologic rehabilitation to design and implement targeted treatment plans that address specific injuries and conditions. Additionally, your understanding of geriatric rehabilitation will guide you in providing compassionate and effective care tailored to the unique needs of older adults.
