Essential Duties and Responsibilities
- Cordially meet with and provide direction for patients and guests.
- Promptly responds to calls/mail in a professional, courteous, and helpful manner.
- Maintain filing system for patient after care needs and possibilities.
- With the assistance of the clinical care team and the patient; formulate Aftercare Plans that will ensure best possible outcomes
- Completes the Referral and Intake Processes accurately as it to pertains to referrals to aftercare referral source
- Meet with business development representatives and referral sources to ensure PCC has resources across the country that meet patient needs
- Observes all confidentiality regulations.
Education and Experience
- Bachelor’s degree in psychology, social work, or related field and or the educational and experience equivelent.
- 2 years’ experience in a mental health/healthcare field preferred.
