Position Overview:

The Nurse Care Manager for Innovative Senior Solutions’ Adult Day Health (ADH) Center is responsible for managing client health services, coordinating care, and leading staff and volunteers. These satellite ADH sites, hosted in partnership with local churches, provide respite care for caregivers of seniors with Alzheimer's and dementia. The Nurse Care Manager ensures the well-being and engagement of clients, compliance with healthcare standards, and fosters community connections and volunteer involvement. Travel to each mobile site is required.

Key Responsibilities:

1. Client Health Services:

• Conduct assessments, manage medications, and ensure a holistic care plan for each client.
• Oversee therapeutic activities, personal care, and support for activities of daily living (ADLs).
• Coordinate healthcare services with client families, physicians, and case managers.

2. Staff and Volunteer Management:

• Supervise, train, and support caregivers, engagement specialists, and volunteers.
• Cultivate a supportive environment by fostering teamwork and collaboration between staff and volunteers.

3. Operational Oversight:

• Manage daily operations, including maintaining required documentation, staff and volunteer schedules, and other tasks as required.
• Maintain a safe and engaging environment for all clients.
• Travel to each mobile site to ensure consistent quality of care.

4. Community Engagement and Volunteer Coalition:

• Cultivate a robust volunteer coalition that engages with center participants, fostering a sense of community.
• Encourage volunteers to bring unique activities and programs that enhance client experiences.

Qualifications:

• Licensed Practical Nurse (LPN) license in Georgia.
• Experience in adult day health services, home health, or elder care.
• Strong leadership, team, and volunteer management skills.
• Knowledge of dementia care, healthcare regulations, and senior care best practices.
• Ability to travel to various mobile ADH sites as required.
