Hospital Liaison Representative
: SUMMARY: The Hospital Liaison Representative is responsible for rounding with the providers of the Stewart Center for Palliative Care to discuss any potential referrals for Hospice Savannah Inc. This includes educating referral sources, patients, and families about hospice home care and inpatient services. Responsible for reviewing all clinical records, reviewing payer sources Scheduling visits when appropriate. QUALIFICATIONS: Graduate of an accredited school of Nursing, Licensed Practical Nurse preferred but not required, with experience in hospice, oncology. Non-clinical must have a High School diploma or equivalent required. Experience in healthcare, business development, outreach or related fields preferred. Knowledge of ICD 9/10 coding and/or medical billing experience preferred. Candidate must have a minimum of three years' work experience in a healthcare environment, experience in Medical Terminology and accurate data entry and typing skills. This position requires excellent verbal, written, and computer skills with proficiency in Word, Excel, and Outlook and a working knowledge of general office equipment, including multi-line phone systems, fax machines, and copiers. Must have strong time management and communication skills and the ability to successfully interact and communicate with staff, patients, families, physicians, medical office staff and the community. Excellent customer service, analytical and critical thinking skills are mandatory. Must have access to a personal vehicle and an operable cell phone. PHYSICAL DEMANDS: Position requires moderate visual concentration, reading, and computer usage for evaluation of patient charts. This position requires the ability to lift to 20-75 lbs. utilizing proper body mechanics and the mobility to bend, twist and stoop, as necessary. This position has daily contact with patients, families, medical facilities, physicians, and hospital staff. This position carries the risk of exposure to infectious diseases and requires skill in standard safety precautions. Must be able to type and assist with other general office work and sit and walk for extended periods. Must possess excellent communication and problem-solving skills and professional demeanor. WORK ENVIRONMENT: This position is part-time with a general schedule from 10am - 3:30 pm, Monday through Friday, with the potential of working full-time at 33 hrs. per week. This position includes a 10a-6:30p rotation on the weekends. Flexibility to work additional days and/or hours on the weekend as needed to include rotation in the admissions department. This position may encounter stressful situations with patients and their families regarding end-of-life care. Additionally, this position may be exposed to infectious diseases requiring use of standard precautions. This position requires occasional travel to various locations between Hospitals.
