As an On-Call Hospice RN at Divine Guardian LLC, you will play a vital role in providing compassionate care to patients in their final stages of life. Join our dedicated team in Tucker, GA, and make a meaningful impact on families during their most challenging times. Responsibilities:
- Provide high-quality, patient-centered nursing care to hospice patients in various settings.
- Conduct comprehensive assessments and develop personalized care plans.
- Administer medications and manage symptom control effectively.
- Collaborate with interdisciplinary teams to ensure holistic care.
- Educate patients and families about end-of-life care options and resources.
- Document patient care and maintain accurate medical records.
- Respond promptly to patient and family needs, ensuring comfort and dignity.
- Participate in on-call rotations to provide support as needed.
- Current RN license in the state of Georgia.
- Minimum of 1 year of nursing experience, preferably in hospice or palliative care.
- Strong assessment, communication, and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate demeanor with a focus on patient-centered care.
- Basic Life Support (BLS) certification required.
- Knowledge of hospice regulations and standards of care.
- Flexibility to work on-call hours as needed.
