The Direct Support Professionals will be responsible for the day/to-day care for the individual(s) served assigned to his/her oversight.

Non-exempt position.

DUTIES AND RESPONSIBILITIES

Acts as a positive role model and communicates effectively.

Works effectively as a team member.

Completes tasks accurately and on time.

Manages a crisis or responds to emergency medical situation.

When appropriate, assists individuals served with activities of daily living (self-administration of medication, hygiene and personal care, nutritional oversight, home duties, supervision on medical appointments and community networking, etc.).

Performs household cleaning and general management duties and environmental checks.

Operates a motorized vehicle and provides transportation for the person served to public settings, provides supports and supervises for safety.

Maintains on an annual basis all mandatory required training requirement with DBHDD and the agency.

Provides positive support and assistance to individual(s) served in achieving goals as outlined in their ISP/BSP/SP.

Effectively communicates with the assigned case manager of the individual(s) served to ensure continuity of care.

Reconciles and documents the individual(s) served spending/grocery money at the beginning and end of each shift.

Records and documents on all required documentation, including but not limited to the Shift Communication log, shift change form, temperature logs, visitor’s log, etc.

Other duties as assigned.

QUALIFICATIONS:

Required : High school diploma or GED and a minimum of one (1) year experience working with individuals with Developmental Disabilities, preferably in a behavior-focused residential home. Must have a valid GA Driver’s License, reliable transportation, a good driving record and be able and willing to drive to appointments, outings, etc. Must have and maintain a phone number in order to be reached. Must be familiar with and able to operate calculator, computer, fax, Hoyer lift assistive equipment, all household appliances.

Job Type: Part-time

Compensation: $9.00/hour
