- Core responsibilities: Provide comprehensive nursing assessment and continuous monitoring of medically complex, post-surgical, and medically unstable medical-surgical patients. Develop and adjust individualized care plans, administer medications and IV therapies, manage wounds and drains, support pain control, and coordinate safe and timely discharges. Deliver patient and family education to promote self-management, seniors’ health literacy, and adherence to treatment plans. Collaborate with physicians, therapists, case managers, and ancillary staff to ensure seamless care transitions and consistent documentation in electronic health records. Remain vigilant for changes in condition, escalate when necessary, and participate in unit-based quality improvement initiatives.
