Case Manager (ICM)

New Horizons Behavioral Health

Columbus, GA

Under general supervision, the candidate will perform social services functions at an advanced level to individuals and their families. Assist in engagement, needs identification, care coordination, referrals and linkage, and follow-up with individuals on assigned caseload. Skills in documentation, communication with individuals, family, and community resources expected. Travel between Columbus, Ga and Americus, Ga required.

Program: Intensive Case Management (ICM)

Job Type: Full-Time (40 hours per week)

Works with the following population: Individuals with severe and persistent mental health diagnosis, developmental disabilities, and/or addictive diseases.

Hours : 8am to 5pm, Monday through Friday.

Salary : $33,000 per year

Minimum Qualifications : High School Diploma with 3-5 years' experience in behavioral health -or- completion of a bachelor's degree in social work, psychology, mental health, sociology, counseling, or a related discipline. If candidate has a Bachelors, it must be in the above named fields to be MHC eligible. Must have a driver's license, a personal insured vehicle, and a clean driving record, as this position transports.
Preferred Qualifications : A bachelor's degree and experience working with individuals with mental disorders and/or addictive diseases is preferred.
