• Registered Nurse (RN) Case Manager – Home Health
• Location: Chatham, Liberty, Bryan, and Effingham Counties, GA
• Schedule: Monday–Friday, 8:30 AM – 5:00 PM
• Employment Type: Full-Time
• Pay rate: $50.00-55.00 an hour paid weekly + mileage reimbursement
• Benefits : available to full-time employees after 90 days of employment

Position Overview

We are seeking a dedicated and experienced Home Health Registered Nurse (RN) Case Manager to provide comprehensive care coordination for high-risk members in a home health setting. This role focuses on delivering patient-centered care, improving outcomes, and ensuring cost-effective treatment plans through collaboration and proactive case management.

Key Responsibilities

• Conduct comprehensive assessments of high-risk patients
• Develop and implement individualized care plans in collaboration with primary care providers
• Continuously evaluate and adjust care plans to ensure appropriate and cost-efficient care
• Coordinate care transitions across healthcare settings
• Collaborate with physicians, social workers, discharge planners, and other healthcare professionals
• Arrange non-medical support services (e.g., housing, transportation) to support treatment compliance
• Utilize specialty resources to optimize patient outcomes
• Maintain accurate documentation of clinical, functional, and financial outcomes
• Identify opportunities for health promotion and disease prevention
• Intervene promptly to prevent or minimize adverse patient outcomes
• Advocate for patients while maintaining strict confidentiality and privacy standards

Qualifications

• Associate’s or Bachelor’s Degree in Nursing or related field
• Active, unencumbered Georgia RN license (required)
• Home Health Experience Required
• Proficiency in OASIS documentation and accuracy
• Experience with HCHB (Homecare Homebase)
• Case Management Certification (preferred)
