Patient Care Manager, Home Health / Registered Nurse

About the Role:
As a Patient Care Manager, you will play a pivotal role in overseeing the clinical operations of our home health services. You will be responsible for ensuring the highest quality of care is provided to our patients while leading and supporting our dedicated team of healthcare professionals.
Key Responsibilities:
- Manage and coordinate patient care services, ensuring compliance with health regulations and standards.
- Supervise and mentor home health staff, fostering a positive and collaborative work environment.
- Conduct regular assessments and develop personalized care plans for patients.
- Act as a liaison between patients, families, and healthcare providers to ensure effective communication and patient satisfaction.
- Monitor patient outcomes and implement improvement strategies as needed.
Qualifications:
- Current and valid RN license in the state of Georgia.
- Minimum of 3 years of nursing experience, with at least 1 year in a home health or similar setting.
- Proven leadership skills with experience in team management.
- Excellent communication and interpersonal skills.
- Strong organizational and problem-solving abilities.
