About the Role
As a valued member of our team, you will conduct thorough health assessments during visits that last up to one hour. You will gather critical information about a member's health history, medication compliance, social environment, and risks, enabling us to deliver exceptional care and close multiple care gaps in one visit. Your responsibilities will include:
- Conducting detailed adult/geriatric assessments, including medical history, diagnosis and treatment plans, health education, physician referrals, and essential documentation in line with Matrix protocols.
- Administering and ordering point-of-care and lab-based tests as necessary.
- Collaborating with healthcare professionals, including physicians, case managers, family members, and caregivers.
- Coordinating with Primary Care Physicians (PCPs) for patient education and follow-up care.
- Delivering services at members' residences, which may include their homes or skilled nursing facilities.

- Flexibility - Schedule visits based on your availability. Assessments take place Sunday through Saturday from 8 am to 7 pm, allowing you to work during the day, evening, or weekends.
- Competitive Pay - Your income is based on the number of assessments completed, with potential earnings ranging from $1,800 to $11,500 per month for those who complete 20 to 100 visits monthly, based on a per-assessment rate of $90 to $115 depending on your state's guidelines.

Qualifications
- Master's Degree in a relevant field or equivalent experience with current Nurse Practitioner licensure.
- Active NP licensure in your state of practice, including prescriptive authority, or the eligibility to obtain it.
- Board certification from AANP, AACN, or ANCC in an appropriate specialty.
- Current BLS, ACLS, or CPR certification.
- Preferred: At least one year of NP experience, though new graduates are encouraged to apply!
- Valid driver's license and automobile insurance per state requirements.
- Proficient computer skills and familiarity with healthcare/medical record software.
- Excellent verbal and written communication abilities.
