About the Role:

The Hospice Admissions Registered Nurse (RN) plays a critical role in providing compassionate and comprehensive care to patients and their families during the transition to hospice services. This position is responsible for conducting thorough patient assessments to determine eligibility and appropriateness for hospice care, ensuring that patients receive timely and personalized support. The RN collaborates closely with interdisciplinary teams, including physicians, social workers, and chaplains, to develop and implement individualized care plans that honor the patient’s wishes and clinical needs. Effective communication and empathy are essential as the RN educates patients and families about hospice philosophy, services, and what to expect throughout the care journey. Ultimately, this role ensures a smooth admission process that upholds the dignity and comfort of patients at the end of life while supporting their families emotionally and practically.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the state of practice.
  • Minimum of 2 years clinical nursing experience, preferably in hospice, palliative care, or related fields.
  • Strong knowledge of hospice care principles, patient assessment, and symptom management.
  • Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare teams.
  • Ability to work independently and manage multiple priorities in a fast-paced environment.

Preferred Qualifications:

  • Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.
  • Experience with electronic medical records (EMR) systems and hospice-specific documentation.
  • Background in community or home health nursing.
  • Training in grief counseling or bereavement support.
  • Bilingual abilities to support diverse patient populations.

Responsibilities:

  • Conduct comprehensive patient assessments in various settings to evaluate eligibility for hospice care.
  • Collaborate with physicians, social workers, and other healthcare professionals to develop individualized care plans.
  • Educate patients and families about hospice services, care expectations, and available resources.
  • Complete all required documentation accurately and timely to ensure compliance with regulatory standards.
  • Coordinate admissions logistics, including scheduling and communication with interdisciplinary team members.
  • Provide emotional support and guidance to patients and families during the admission process.
  • Maintain up-to-date knowledge of hospice policies, procedures, and best practices.
  • Participate in ongoing training and quality improvement initiatives to enhance patient care.

The Hospice Admissions RN utilizes clinical assessment skills daily to evaluate patient conditions and determine hospice eligibility accurately. Strong communication skills are essential for educating patients and families about hospice care options and addressing their concerns with empathy and clarity. Organizational skills enable the RN to manage documentation, coordinate admissions, and collaborate efficiently with interdisciplinary teams. Critical thinking and problem-solving abilities support the development of individualized care plans that meet complex patient needs. Additionally, cultural competence and emotional intelligence are applied consistently to provide respectful, patient-centered care that honors diverse backgrounds and end-of-life preferences.
