Registered Nurse - Hospice Admissions RN

Responsibilities:
- The Admissions RN Will Meet with Referral Sources, Patients, And Families to Determine Eligibility for Hospice Services
- Provide Education on Hospice Services and Care Plans
- Coordinate Admissions and Ensure Smooth Transition into Hospice Care
- Maintain Accurate Documentation and Communication with Clinical Team
- Deliver Strong Clinical and Customer Service Support
- Hours Are 10:00 AM-8:00 PM
- 40 Hours Per Week
Week 1 - Sun-Tues-Wed-Thursday
Week 2 - Tues-Wed-Fri-Saturday
Requirements:
- Registered Nurse in the State Of Georgia
- 1 Year of Hospice, Home Health, or Med-Surg Experience Preferred
- Strong Clinical and Customer Service Skills
