Field Assessment Nurse-RN
Responsibilities: * Field Supervisory Visits:
- Conduct regular visits to clients' homes to ensure the quality of care provided by caregivers meets company standards.
- Evaluate caregiver performance and provide constructive feedback and support as necessary.
- Collaborate with other healthcare professionals involved in the clients' care to ensure continuity and quality of services.
- Perform comprehensive medical assessments for new clients to determine their healthcare needs and develop personalized care plans.
- Communicate assessment findings and care plans effectively with the interdisciplinary team, including caregivers, clients, and their families.
- Maintain accurate and timely documentation of all client assessments, supervisory visits, and training sessions.
- Generate reports as required by regulatory agencies and company policies.
- Valid RN license in the state of Georgia.
- Strong clinical assessment skills and the ability to develop individualized care plans.
- Excellent communication and interpersonal skills, with the ability to interact effectively with clients, caregivers, and other healthcare professionals.
- Demonstrated ability to work independently and as part of a multidisciplinary team.
- Proficiency in computer skills, including electronic medical records systems.
- Current CPR/1st Aid certification.
