Responsibilities for a Social Worker / Discharge Planner with Phoebe Allentown:

Responsible for providing comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs to allow for successful discharge to appropriate level of care. Works with interdisciplinary team to plan for discharge and set up appropriate community services; works with team to decrease potential for hospital readmissions. Serves as a liaison between the facility and the patient and their family. Provides various social work services to patients/residents; coordinates work within department as well as with other departments; assists with completion of resident MDS process; follows through with assignments; provides periodic reports to supervisor and others as requested.

Requirements for a Social Worker / Discharge Planner with Phoebe Allentown:

1. The ability to deliver excellent customer service to all residents, families, and coworkers.
2. B.A. or B.S. degree in social work or related field
3. Minimum one (1) year experience in discharge planning, long term care nursing, social work, or related discipline
4. Competence with MDS completion; familiarity with interdisciplinary care planning. Experience with discharge planning. Working knowledge of Pennsylvania Department of Health long-term care regulations.
