Weekend Baylor Home Health Physical Therapist - Full-Time Benefits
Join our dynamic healthcare team as a Weekend Baylor Home Health Physical Therapist in beautiful coastal Connecticut. This role offers an exceptional work-life balance with weekdays free while earning full-time benefits working weekends. You'll make a meaningful impact helping patients regain independence in the comfort of their homes while enjoying competitive compensation ($115,000-$120,000 annually).
Core Responsibilities:- Conduct comprehensive patient evaluations and develop personalized treatment plans
- Deliver evidence-based therapeutic interventions including manual therapy, gait training, and therapeutic exercise
- Perform detailed home safety assessments and recommend adaptive equipment
- Provide expert patient/caregiver education on exercises, fall prevention, and safe mobility
- Maintain accurate documentation meeting Medicare/Medicaid requirements
- Collaborate with interdisciplinary healthcare teams
- Manage a daily caseload of 5-7 patients within assigned territory
- Master's degree in Physical Therapy required
- Current state Physical Therapy license
- Minimum 1 year home health experience
- Valid driver's license and reliable transportation
- Current CPR certification
- Strong communication and interpersonal skills
- Ability to work independently and manage time effectively
