FULL-TIME NURSE PRACTITIONER (NP)

Job Responsibilities:

The Nurse Practitioner (NP) is responsible for performing all OASIS SOC assessments in the assigned area. This includes developing a plan of treatment and providing advanced clinical assessment and patient care services in accordance with the plan of treatment. Care is delivered at the client’s home.

• Monitor patient progress and identify any changes in status, acting on those changes to ensure patient comfort and safety.
• Perform patient assessments and provide appropriate clinical care while utilizing age-specific and universal precaution practices at all times.
• Provide patient and family education and training.
• Actively demonstrate a working knowledge of advanced nursing theory, techniques, principles, and practice to holistically care for the patient.
• Communicate with all staff, patients, and families with excellent written and verbal skills.
• Comply with all facility policies and procedures while upholding the expectations of MedSide Healthcare.

Job Qualifications:

• Graduate of an approved Nurse Practitioner program.
• Current NP license/certification, with no sanctions, restrictions, or provisions, in the state of Georgia.
• One year of related experience in a clinical environment.
• Experience with OASIS and home health preferred.
