Physical Therapist Job Details

Physical Therapist Qualifications

• Active Physical Therapist license in the state of New Hampshire
• New graduates are welcome
• Rehabilitation Therapy certification
• SNF (Skilled Nursing Facility) experience preferred
• COVID test required within 72 hours prior to start date; results available 48 hours before start
• Ability to provide documentation: resume, license verification, references, background checks, and proof of 8-hour dementia training

Physical Therapist Responsibilities

• Deliver high-quality physical therapy services to patients in a hospital setting
• Conduct comprehensive patient assessments and develop individualized treatment plans
• Collaborate with interdisciplinary healthcare teams for optimal patient outcomes
• Document patient progress and maintain accurate clinical records
• Educate patients and families on therapy goals and home exercise programs
• Participate in team meetings and contribute to a positive, patient-centered healthcare environment
