Job Overview

Providence Healthcare of Thomaston is seeking a compassionate and dedicated Occupational Therapist (OT) to join our in-house rehabilitation team. We are hiring Full-Time, Part-Time, and PRN (Per Diem) Occupational Therapists to provide high-quality occupational therapy services in our skilled nursing and subacute rehabilitation setting.

As an Occupational Therapist, you will evaluate residents, develop individualized treatment plans, and provide evidence-based interventions to improve independence, safety, and functional performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs). You will play a key role in helping residents regain meaningful participation in daily life following illness, injury, surgery, or hospitalization.

You will collaborate with an interdisciplinary team committed to delivering resident-centered care and achieving optimal functional outcomes.

Essential Duties and Responsibilities
  • Perform comprehensive occupational therapy evaluations and develop individualized treatment plans based on clinical findings, physician orders, and resident goals

  • Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, strength, coordination, balance, cognition, and fine motor skills

  • Address cognitive, perceptual, and functional deficits impacting independence and safety

  • Train residents in self-care tasks, adaptive techniques, energy conservation, and compensatory strategies

  • Recommend, fabricate, and train residents in the use of adaptive equipment, splints, and assistive devices as appropriate

  • Monitor resident progress, reassess outcomes, and modify treatment plans to maximize functional independence

  • Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, recertifications, and discharge summaries in accordance with Medicare, Medicaid, state, federal, and facility guidelines

  • Collaborate with physicians, nursing staff, physical therapists, speech-language pathologists, dietitians, and other interdisciplinary team members to coordinate resident care

  • Educate residents, families, and caregivers on safety techniques, home programs, fall prevention, and adaptive strategies

  • Participate in interdisciplinary care planning meetings, family conferences, and discharge planning

  • Maintain compliance with all professional, regulatory, and facility policies and standards

  • Promote a positive, resident-centered rehabilitation environment focused on quality outcomes and clinical excellence

QualificationsRequired
  • Graduate of an accredited Occupational Therapy program

  • Current Georgia Occupational Therapist (OT) license in good standing

  • Current CPR certification or willingness to obtain upon hire

  • Strong clinical assessment, communication, and documentation skills

  • Ability to work effectively within an interdisciplinary healthcare team

Preferred
  • Experience in skilled nursing, subacute rehabilitation, long-term care, or inpatient rehabilitation

  • Familiarity with Medicare documentation and reimbursement guidelines

  • Experience working with geriatric and medically complex populations

  • Knowledge of evidence-based occupational therapy practices, adaptive equipment, and cognitive rehabilitation
