Job Description

Description

The Youth Project Manager at Partners for HOME plays a vital role in advancing the organization’s mission by expertly managing and overseeing the Youth Homelessness System Improvement (YHSI) Grant. Reporting to the Associate Director of Special Populations, this position is ideal for a skilled, energetic professional with strong expertise in data management, system administration, and community collaboration.

The Youth Project Manager will be responsible for the full implementation of the YHSI Grant, including collecting and analyzing data on at-risk youth and youth experiencing homelessness; improving coordination, communication, operations, and administration of homelessness assistance projects; and supporting prevention and diversion strategies as outlined in the grant milestones.

Additionally, this role will lead the development and maintenance of training materials, manage system updates, and troubleshoot technical issues, while fostering strong relationships with internal teams and external partners. This position is critical to ensure that data-driven insights advance the goals of the YHSI Grant and strengthen services to youth in the community, requiring a blend of HMIS expertise, strategic oversight, and effective stakeholder engagement.

Key Responsibilities
  • Develop and co-lead the Fulton County and City of Atlanta Youth Homelessness Service Providers Consortium (YSP) with support from the Fulton County CoC.
  • Identify and invite nonprofit organizations in the City of Atlanta to join the YSP and continue to reach out to new groups that help youth experiencing homelessness.
  • Plan and run quarterly YSP meetings (both online and in person) to talk about challenges in serving youth, share resources and ideas, take part in activities, and include feedback from the Youth Advisory Boards (YABs).
  • Work with the Fulton County CoC and other partners to organize training for YSP member organizations.
  • Support the City of Atlanta YABs by giving advice on activities and helping them plan how to keep their work going after the grant ends.
  • Hold and co-facilitate monthly meetings with the City of Atlanta YAB.
  • Set up and manage self-governance training courses for all YAB members, with topics chosen by the youth.
  • Bring together the YABs, YSP members, and youth from the community for meetings every quarter.
  • Work with YSP and research partners to plan and run workgroups that will create a System and Asset Map during the grant period.
  • Help research partners plan and carry out program reviews to see if goals are met and find out if the project is helping youth effectively.
  • Work with research partners to build a plan to measure and improve current and future projects that help youth and other groups identified by the grant.
  • Partner with data and research teams to improve HMIS (ClientTrack) so it can better share information with systems used by YSP organizations.
  • Develop and carry out plans to use data to learn about youth homelessness, youth needs, and ways to keep improving services in the community.
  • Create Coordinated Entry System (CES) policies for YSP organizations working with youth in both the Fulton County and City of Atlanta areas.
  • Work with the Atlanta CoC to provide mobile Coordinated Entry services that reach youth in all parts of the service area.
  • Team up with YSPs, YABs, and research partners to design and use a youth-specific assessment tool in both CoCs.
  • Work with the Fulton County and City of Atlanta CoC leads to keep checking and improving the youth Coordinated Entry process.
  • Visit CoC youth service providers regularly to stay up to date on the programs and services they offer.
