Radius Rule: Must be willing to travel up to 75 miles from home clinic

Requirements:
- Minimum 1 year of recent Adult Home Health or Hospice experience
- Must have recent HomeCare HomeBase (HCHB) experience (within last 6 months)
- Must be comfortable with daily travel (up to 75 miles)
- Reliable transportation, valid driver’s license, and auto insurance required
- Must be able to meet weekly productivity expectations (30 points / ~40 hours)
- No first-time home health candidates
- Must be open to on-call and weekend rotation
Certifications:
- Active LPN/LVN license (must be in hand)
- BLS (CPR) required
Responsibilities:
- Provide and coordinate home health nursing care based on physician orders
- Perform patient visits including start of care, wound care, and follow-ups
- Manage 5–10 patients per day (approx. 5–8 visits/day)
- Complete documentation within 24 hours using HomeCare HomeBase
- Travel between patient homes and care facilities within assigned territory
- Participate in on-call rotation (1 day/week and every 3rd weekend)
- Support and supervise Home Health Aides as needed
