Position Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. In this role, you will play a vital part in delivering exceptional patient care, providing clinical support, and assisting with administrative tasks to ensure the smooth operation of healthcare services. You will work closely with providers, nurses, pharmacies, hospitals, and other healthcare professionals to support patient treatment and recovery.

This position may require remote work and is subject to all current CMS vaccine mandates.

Key Responsibilities:

Patient Care & Clinical Support:

Prepare patients for exams and treatments by gathering medical history, vital signs, medications, and allergy information.

Administer medications, injections, wound care, and assist with procedures such as EKGs and GYN exams under provider supervision.

Monitor and report any adverse reactions to medications or treatments.

Educate patients on medications, treatment plans, and disease management strategies.

Assist with medication assistance program refills.

Collect and process lab samples (urine, blood, sputum) and perform routine laboratory tests.

Administrative & Coordination Duties:

Act as a liaison between patients and providers to ensure effective communication.

Assist with scheduling tests, treatments, and specialist referrals.

Maintain clinical supplies, exam rooms, and medical equipment, ensuring everything is stocked and operational.

Track and log immunizations, drug samples, and vaccine inventory.

Ensure compliance with HIPAA and OSHA regulations.

Support Patient-Centered Medical Home (PCMH) initiatives and meaningful use criteria.

Quality & Compliance Responsibilities:

Participate in quality assurance initiatives, professional development, and emergency preparedness training.

Assist in maintaining regulatory requirements for laboratory operations.

Monitor and document patient progress, chronic disease care plans, and test results.

Foster a workplace culture that emphasizes respect, teamwork, and compassionate care.

Qualifications & Skills:

Strong knowledge of clinical procedures, medical equipment, and safety protocols.

Excellent problem-solving, communication, and patient care skills.

Ability to remain calm and professional in emergency situations.

Proficiency in medical record documentation and basic arithmetic.

Experience using Electronic Health Records (EHR) systems.

Ability to work collaboratively with a multidisciplinary healthcare team.
