We are seeking a Family Nurse Practitioner of Physician Assistant to work full-time at Tanner Urgent Care clinics.

Job Duties & Responsibilities
  • In collaboration with a physician(s), the NP/PA performs triage, physical exams, collects and documents data, orders laboratory studies, diagnostic procedures, prescribes appropriate interventions and medications, coordinates the care of patients, and provides direct patient care services.
  • Analyze clinical practice patterns to provide the best medical care and to increase effectiveness and efficiency.
Job Eligibility Requirements

Education & Licensure
  • Nurse Practitioners: Requires completion of an accredited Nurse Practitioner program, with a master's degree in nursing.
  • Physician Assistants: Completion of an accredited Physician Assistant program.
  • Must be board certified.
  • GA licensure required.
Experience
  • 1+ years of APP experience strongly preferred.
Example Schedule Option
  • Week 1: Monday & Tuesday, 8a - 8p (or until last patient seen); Sat/Sun 8a - 5p (or until last patient)
  • Week 2: Wednesday, Thursday & Friday, 8a - 8p (or until last patient seen)
  • Locations include Tanner Urgent Care/Carrollton, Tanner Urgent Care/Villa Rica, and Tanner Urgent Care/Bremen
  • Scheduled at one clinic site daily

Functions

Area of Responsibilities

* Patient Care: Performs evaluations, provide treatment and counsel patients. Order and interpret test results and recommend treatment of patients. Administer therapeutic procedures. Prescribe therapy or medication with physician approval. Provide physicians with assistance during surgery or complicated medical procedures. Demonstrates specialty and population specific competencies. Demonstrates an ability to effectively communicate based on specific specialty and population. Performs other duties as assigned by the supervising physician.

* Teamwork: Works well with others and functions as a team player. Offers ideas and solutions for issues that affect the team and work area. Participates in the training and skills development of new clinical staff. Assists as needed.

* Quality: Maintains evidence based medical care and clinical charting. Participates with clinical tracking and feedback. Integrates with clinical and administrative teams to administer high quality care.

* Development: Maintain and update level of skill for pertinent medical assistant duties. Participates in continuing education. Participates in the development of new programs (ex: Process Improvement, Quality Initiatives).

* Customer Service: Participates in patient satisfaction initiatives. Demonstrates excellent customer service skills. Communicates effectively with patients and management regarding service recovery opportunities. Works together with other team members to achieve Office and System customer service goals.

PATIENT CENTERED MEDICAL HOME RELATED DUTIES AND RESPONSIBILITIES*Access to Care: Triages patients who may need urgent or emergent care. Provides clinical advice by phone during office hours within designated time frame. Documents clinical advice in the medical record. Utilizes and provides continuity of medical record information for care and advice when the office is not open. Documents after hours clinical advice in patient records. Asks patients about care obtained outside the practice. Provides access to evidence based care and self-management support. Participates in a structured communication process, i.e. huddles. Authorizes and uses standing orders for important conditions. Provides extended hours as needed to accommodate patient population.
  • Identify and Manage Patient Populations: Collects and reviews clinical data, i.e. problem list, allergies, BP, height and weight, tobacco use, current medications. Completes comprehensive health system. Provides input for three different preventive care services. Provides input for three different chronic care services.
  • Plan and Manage Care: Implements evidence based guidelines. Provides a written plan of care for the patient family. Assesses and addresses barriers when the patient has not met treatment goals. Identifies patients who might benefit from additional care management support. Reviews and reconciles medications. Provides information about new prescriptions. Assesses understanding about medication and barriers to adherence. Documents over the counter medication, herbal therapies, and supplements. Generates and transmits prescriptions to pharmacies. Performs patient specific drug-drug and drug-allergy interactions.
  • Self-Care Support and Community Resources: Provides educational resources to assist with self-management and support. Develops and documents self-management plans and goals in collaborations with the patient, family. Documents self-management abilities and provides tools to record self-care. Counsels patients' families to adopt healthy lifestyles.
  • Track and Coordinate Care: Tracks referrals to patients' families. Tracks labs and imaging tests until results are available, flagging and following up on overdue results. Notifies patients' families of abnormal lab test results, as needed. Establish and document agreements with specialists in the medical record if co-management is needed. Collaborates with the patient family to develop a written care plan for patients transitioning from pediatric care to adult care.

Compliance Statement

Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.

Education: Master's Degree

Experience: Six months of related experience

Licenses and Certifications

* GEORGIA REGISTERED NURSE LICENSE OR MULTISTATE NURSING LICENSE

* GEORGIA NURSE PRACTITIONER LICENSE

* HEALTHCARE PROVIDER (CPR)

Supervision

Regularly uses assistance and leads the work of one to four co-employees, may instruct new employees and correct work routines, checks results and may discuss with employee's superior.

Qualifications

* Graduate of a recognized program for Nurse Practitioner training (MSN).

* Current certification as a Nurse Practitioner in Georgia

* ACLS recommended.

* Able to communicate effectively with medical and hospital staff and various populations in clinics and public screening events.

* Must possess the sense or discipline to work in accordance with accepted clinical standards.

* Must maintain high degree of confidentiality.

* Must possess the cognitive ability to understand patient medical records, physician orders and medical orders to be communicated to patient, hospital and/or other physician.

* Must be capable of dealing tactfully and effectively with patients, family members, other employees and physicians

* Must meet credentialing criteria.

* Prior experience in a like specialty preferred.

* Additional certification will be required for some specialties (i.e. ACLS, PALS)

Definitions

Nurse practitioner provides patient services through the performance of patient evaluation, assessment and treatment in the medical office setting as allowed by state licensure laws under the supervision of the medical director. The nurse practitioner works closely with the clinic staff, hospital departments and community agencies.

Continually works with reports, records, plans, and programs of a major functional area of the organization where integrity is required to safeguard the organization's position. Duties may involve the preparation of data on which the administration bases important decisions and are highly confidential.

Working Conditions: Moderate - (About 50% of the day) Involved in exposure to dirt, odors, noise, or some work is performed with exposure to temperature/weather extremes/occupational risk. Limited probability of coming into contact with blood borne pathogens, other potentially infectious diseases, or biomedical/bio-hazardous materials.
