Lead Nephrologist (Brookhaven)

Roles & Responsibilities

1. Provide comprehensive nephrology care to patients with acute and chronic kidney diseases at home and in dialysis facilities
2. Collaborate with interdisciplinary teams to develop and implement value-based care initiatives aimed at improving patient outcomes and reducing healthcare costs
3. Utilize evidence-based practices and clinical guidelines to optimize the management of kidney disease and associated comorbidities
4. Engage in patient education and shared decision-making to empower patients in managing their kidney health
5. Utilize technology and data analytics to track and monitor patient outcomes, identify areas for improvement, and drive continuous quality improvement
6. Participate in multidisciplinary care conferences and meetings to review complex cases and collaborate on treatment plans
7. Drive key clinic metrics impacting kidney care, such as Home Dialysis rate, permanent access, planned dialysis start in outpatient settings, and conservative kidney management

Position Requirements

1. Medical degree from an accredited medical school
2. Board certification in Internal Medicine and Nephrology
3. Current state medical license and DEA registration without restrictions
4. Strong clinical skills and experience managing patients with kidney diseases across inpatient, outpatient, and dialysis settings
5. Commitment to patient-centered care and shared decision-making
6. Excellent communication and interpersonal skills for effective collaboration with patients, families, and healthcare teams
7. Experience with value-based care models, quality improvement, and population health management preferred
