Part-Time Medical Director - Skilled Nursing Facility - Flexible - Savannah
What You'll Be Doing
The Medical Director plays a key leadership role in Fortis Care's clinical operations within a skilled nursing facility. The Medical Director provides oversight, mentorship, and guidance to ensure the delivery of high-quality, patient-centered care across post-acute and long-term care settings.
This position combines direct patient care with system-level leadership. The Medical Director partners closely with facility administrators, nursing leaders, and advanced practice providers to standardize care processes, drive quality improvement, and uphold clinical excellence across all Fortis Care partner sites. Medical Directors typically spend 2 half days per week on site and participate in monthly quality improvement meetings.
Key Responsibilities
Clinical Leadership
- Direct and oversee the delivery of medical care for residents in post-acute and long-term care settings.
- Lead and mentor interdisciplinary care teams, setting clinical standards for exceptional patient care.
- Support nurse practitioners and clinicians through case reviews, chart audits, and feedback sessions.
- Serve as a clinical resource for complex cases and end-of-life care decision-making.
Care Process & Quality Improvement
- Implement evidence-based clinical and administrative protocols.
- Lead quality improvement and performance initiatives (e.g., QAPI programs).
- Monitor and ensure compliance with state, federal, and payer regulations.
- Analyze outcomes and identify opportunities to improve safety, efficiency, and resident experience.
Education & Communication
- Provide ongoing education to facility and Fortis Care staff on clinical best practices, documentation, and regulatory standards.
- Promote effective communication between care teams, residents, and families.
- Foster a culture of learning, accountability, and professional growth among providers.
Risk Management & Compliance
- Guide the development and implementation of infection control and risk mitigation strategies.
- Support medical and ethical decision-making around advance care planning, palliative care, and patient self-determination.
Community & Staff Advocacy
- Serve as an advocate for resident rights and quality of life.
- Engage in community and facility initiatives that promote population health and care innovation.
Skills & Abilities
- Proven leadership skills with the ability to mentor and influence interdisciplinary teams.
- Strong clinical background with expertise in geriatrics, internal medicine, or family medicine.
- Excellent communication and relationship-building skills with staff, patients, and partners.
- Proficiency in regulatory compliance, QAPI, and data-driven performance improvement.
- Compassionate, mission-driven approach to patient care.
Qualifications
Education
- MD or DO degree from an accredited medical school.
Experience:
- Minimum of 3 years of clinical experience in geriatrics, internal medicine, or family medicine.
- At least 2 years of leadership experience in post-acute, long-term care, or related healthcare settings.
Licensure:
- Active, unrestricted state medical license.
- Board certification in Geriatrics, Internal Medicine, or Family Medicine.
- Active DEA license (Class II–V).
- Current CPR certification.
Preferred Qualifications:
- Experience in wound care, hospice, or long-term care medical direction.
- Bilingual (English/Spanish) a plus.
