Hospice Liaison
We are a growing hospice organization serving 14 counties across the Metro Atlanta area. With a current census of 35 patients and a dedicated team of 3 RNs, 2 LPNs, and 1 existing liaison, we are ready to expand and continue building a compassionate, patient-focused program.
The Hospice Liaison plays a key role in developing and maintaining relationships with referral sources, patients, and families to support growth and admissions. This individual will represent the organization within the community and healthcare facilities to ensure patients and families receive timely, quality hospice care.
Compensation & Benefits:
- Competitive salary up to $110,000 , based on experience.
- Bonus structure: $1,000 upon meeting monthly admission target; $500 per admission beyond that within the same month.
- Mileage reimbursement: $0.50 per mile.
- Health insurance: Provided through United Healthcare.
- PTO: 3 weeks annually.
Schedule:
Monday–Friday (flexible hours; referral activity takes priority).
Responsibilities:
- Build and maintain relationships with hospitals, physicians, skilled nursing facilities, and other referral sources.
- Drive hospice admissions by identifying and engaging new referral opportunities.
- Achieve and exceed a target of 5–6 new admissions per month, with growth goals of 7–10.
- Participate in daily morning huddles at 8:15 AM EST to review referrals and priorities.
- Travel throughout assigned counties to meet referral partners and patients (mileage reimbursed).
- Collaborate with clinical and administrative staff to ensure smooth coordination of care.
- Maintain accurate referral documentation (no required CRM—use your preferred system).
- Represent the hospice program professionally and compassionately in all interactions.
Requirements:
- Proven success in healthcare sales, marketing, or liaison work—hospice or home health experience preferred.
- Existing book of business with referral sources is highly desirable.
- Must reside within or near the Metro Atlanta area and be willing to cover at least 2–3 local counties.
- Valid driver’s license, reliable vehicle, and active auto insurance.
- Strong communication, relationship-building, and organizational skills.
