- Provide skilled nursing care to patients in their homes in accordance with the plan of care
- Assist with patient assessments and report findings to the supervising RN
- Administer medications and treatments as prescribed
- Monitor and document patient conditions, progress, and responses to care
- Observe and report any changes in patient condition to the RN or supervisor
- Educate patients and families on medications, treatments, and daily care needs
- Maintain accurate and timely documentation in compliance with agency and state requirements
- Follow all infection control, safety, and home healthcare protocols
- Current Licensed Practical Nurse (LPN) license in the state of Georgia
- Previous home healthcare experience REQUIRED
- Knowledge of basic nursing practices and home health procedures
- Excellent interpersonal, communication, and patient care skills
- Ability and willingness to travel to Atlanta, Cobb, Gwinnett, Henry, and Clayton Counties
- Valid driver’s license and reliable transportation
- Current First Aid and CPR certification
- Strong organizational skills and attention to detail
