Coordinate patient care from admission to discharge, develop care plans, collaborate with healthcare teams, educate patients and families, and ensure safe, efficient, and cost-effective outcomes.

Requirements

Requirements:

• Active RN license (state or compact)

• 3–5 years clinical experience (acute care preferred)

• Case management knowledge (CCM preferred)

• Strong communication and organizational skills

• EMR/EHR experience
