About Your Role

As a Nurse Practitioner with Matrix Medical Network, you will conduct in-depth health assessments during visits that last up to one hour. You will have the opportunity to make a significant impact on patients' lives by reviewing their health history, medication adherence, and social environments. Here's what you'll do:

• Perform adult and geriatric assessments, including medical history, diagnoses, treatment plans, health education, and appropriate referrals.
• Administer and order necessary lab tests and point-of-care assessments.
• Collaborate with physicians, case managers, social workers, and family members to ensure comprehensive care.
• Provide services directly in the patient's home or skilled nursing facilities.

Qualifications

• Master's Degree or equivalent experience with completed NP licensure.
• Current NP licensure in your practicing state with prescription authority.
• AANP, AACN, or ANCC board certification in your specialty.
• Current BLS, ACLS, or CPR certifications.
• While 1 year of NP experience is preferred, we welcome new graduates to apply!
• Valid driver's license and automobile insurance that meets state requirements.
• Strong computer proficiency and excellent communication skills.
