The RN will provide skilled nursing services in a home setting, supporting clients with clinical needs while also conducting assessments and developing individualized care plans. - Current license as a Registered Nurse (RN) in the state(s) of practice
- CPR certification
- Demonstrated knowledge of safety and infection control practices
- Perform skilled nursing procedures in the home, including:
- IV infusions
- Wound care
- Medication setup and management
- Chronic disease management (e.g., diabetes, CHF, COPD)
- Conduct initial and ongoing client assessments
- Develop, implement, and update individualized care plans
- Monitor client progress and communicate changes to appropriate parties
- Educate clients and family members on care needs and disease management
- Ensure compliance with all state regulations and company policies
- Maintain accurate and timely clinical documentation
- $45-50 per hour depending on experience
