Your day as a Licensed Practical Nurse (LPN)Able Mabel is seeking a detail-oriented and compliance-driven Licensed Practical Nurse (LPN) to support participants enrolled in Georgia Medicaid waiver programs, including CCSP, SOURCE, ICWP, NOW/COMP, and Structured Family Caregiving (SFC). In this vital role, you will work under the supervision of a Registered Nurse (RN) to ensure the delivery of person-centered care and strict adherence to documentation accuracy and EVV compliance. Your responsibilities will include providing skilled nursing services within your scope, implementing and monitoring Individual Service Plans (ISP), and completing Medicaid-compliant documentation. Additionally, you will monitor participant health status, report changes to the RN, and support caregivers and Direct Support Professionals (DSPs) with delegated nursing tasks. You will also play a key role in care coordination, quality assurance efforts, and assisting with audit readiness and regulatory compliance, contributing significantly to the well-being of our participants.
Are you a good fit for this Licensed Practical Nurse (LPN) role?To excel as a Licensed Practical Nurse (LPN) at Able Mabel, candidates must possess an active Georgia LPN license in good standing and have a minimum of one year of nursing experience, preferably in home health or community-based settings. Familiarity with Medicaid documentation standards is essential, as is experience with Electronic Visit Verification (EVV) systems to ensure accurate compliance and documentation practices. Strong attention to detail and a compliance-focused mindset are critical for successful execution of individual service plans, documentation, and care coordination. Additionally, reliable transportation is necessary for conducting community visits to support participants effectively.
