The role includes clinical supervision responsibilities for Physical Therapy Assistants and Home Health Aides as assigned. Candidates must be prepared to complete home visits and document patient progress according to state and agency requirements.

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association or equivalent accrediting body.
• Current, active, and unencumbered Physical Therapist license for the state of assignment prior to start.
• Basic Life Support (BLS) certification from American Heart Association or American Red Cross may be required.
• Demonstrated ability to perform patient evaluations, develop and update plans of care, and document clinical findings in the patient health record.
• Reliable transportation, valid driver's license, and agency-required liability insurance for performing home visits.
• Experience supervising or delegating to Physical Therapy Assistants and Home Health Aides preferred.
• Familiarity with electronic documentation, basic computer navigation, email, and online training systems.
• State- or client-specific compliance with licensing, supervision, and scope-of-practice regulations as required by the assignment.

Preferred Qualifications

• 2–3 years of recent physical therapy experience, preferably in home health or skilled home-care environments.
• Prior travel or agency-based experience supporting continuity of care across multiple patients and settings.
• Experience conducting home environment evaluations and providing family or caregiver education.

Assignment Details

• Shift: Evenings, 5 shifts per week, 8 hours per shift.
• May be required to perform home visits across the service area. Reliable transportation is required.
