About the Role:

The Palliative Care Nurse Practitioner (NP) specializing in home visits plays a critical role in delivering compassionate, patient-centered care to individuals with serious, chronic, or life-limiting illnesses in the comfort of their own homes. This position focuses on managing complex symptoms, coordinating interdisciplinary care, and supporting patients and their families through advanced illness trajectories. The NP will assess physical, emotional, and psychosocial needs, develop and implement individualized care plans, and provide education on disease management and end-of-life options. By working closely with physicians, social workers, and other healthcare professionals, the NP ensures seamless communication and continuity of care. Ultimately, this role aims to enhance quality of life, alleviate suffering, and empower patients and families to make informed decisions aligned with their values and goals.

Minimum Qualifications:

  • Current and unrestricted Nurse Practitioner license in the United States.
  • Certification as a Nurse Practitioner with a focus on adult-gerontology or family practice.
  • Certification in Palliative Care (e.g., AAHPM or HPNA certification) or eligibility to obtain within a specified timeframe.
  • Minimum of 2 years clinical experience in palliative, hospice, or related nursing practice.
  • Valid drivers license and reliable transportation to conduct home visits.

Preferred Qualifications:

  • Experience working in home-based care or community health settings.
  • Advanced training or coursework in pain management and symptom control.
  • Familiarity with electronic health record systems and telehealth technologies.
  • Strong background in interdisciplinary team collaboration and care coordination.
  • Bilingual abilities or experience working with diverse patient populations.

Responsibilities:

  • Conduct comprehensive patient assessments during home visits, including physical exams and symptom evaluation.
  • Develop, implement, and adjust individualized palliative care plans in collaboration with patients, families, and interdisciplinary teams.
  • Manage complex symptoms such as pain, dyspnea, nausea, and psychological distress using evidence-based interventions.
  • Provide education and counseling to patients and caregivers regarding disease progression, treatment options, and advance care planning.
  • Coordinate care with primary care providers, specialists, hospice services, and community resources to ensure holistic support.
  • Document all patient interactions, care plans, and clinical outcomes accurately and timely in electronic health records.
  • Advocate for patient needs and preferences while respecting cultural, spiritual, and ethical considerations.
  • Participate in quality improvement initiatives and stay current with best practices in palliative care.
