Hospice Visit LPN evenings Weds-Sun

Snellville, GA

About Company:

Blue Summit Hospice and Palliative Care was founded based on the knowledge that great clinical care can only be achieved by a great clinical staff. Our company was started by clinicians with hands-on experience working in end-of-life care. And our goal is not only to provide the best possible care but also to be the best possible place to work.

We place as much focus on staff training and retention as we do on our patient care. And that's how it should be! End-of-life care requires a sincere commitment to meeting the unique needs of the patients and families we serve. Blue Summit has that same level of commitment to our staff.

If you are a clinical end-of-life professional or simply want more information about our services, reach out to us, and we'll be happy to explain the Blue Summit difference.

About the Role:

The Hospice Visit Licensed Practical Nurse (LPN) working evening shifts from Wednesday through Sunday plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses. This position focuses on providing skilled nursing care, symptom management, and emotional support to patients and their families in their homes or care facilities during the evening hours. The LPN will collaborate closely with interdisciplinary hospice teams to ensure that care plans are effectively implemented and adjusted according to patient needs. By maintaining accurate clinical documentation and communicating changes in patient status promptly, the LPN helps to optimize quality of life and comfort for hospice patients. Ultimately, this role is essential in supporting the dignity and well-being of patients during their end-of-life journey while providing reassurance and guidance to their loved ones.

Minimum Qualifications:

  • Current and valid Licensed Practical Nurse (LPN) license in the state of practice within the United States.
  • Minimum of one year of clinical nursing experience, preferably in hospice, palliative care, or home health settings.
  • Basic Life Support (BLS) certification from an accredited organization.
  • Ability to work independently during evening shifts with strong time management and organizational skills.
  • Reliable transportation and willingness to travel to patient homes or care facilities within the assigned geographic area.

Preferred Qualifications:

  • Previous hospice or palliative care experience.
  • Certification in Hospice and Palliative Nursing (CHPN) or similar specialty certification.
  • Experience with electronic medical records (EMR) systems and documentation.
  • Strong interpersonal and communication skills tailored to sensitive end-of-life care situations.
  • Familiarity with pain and symptom management protocols specific to hospice care.

Responsibilities:

  • Conduct comprehensive nursing assessments and monitor patients’ physical and emotional conditions during evening visits.
  • Administer medications, treatments, and symptom management interventions as prescribed by physicians and hospice protocols.
  • Provide education and emotional support to patients and family members regarding disease progression, care techniques, and available resources.
  • Collaborate with interdisciplinary team members including physicians, social workers, and chaplains to coordinate holistic care plans.
  • Document all nursing care, patient responses, and any changes in condition accurately and timely in compliance with regulatory standards.
  • Respond promptly to patient and family concerns or emergencies during scheduled visits, escalating issues as necessary.
  • Ensure adherence to infection control policies and maintain patient safety during all visits.

Skills:

The required skills enable the LPN to perform thorough patient assessments and deliver precise nursing interventions that alleviate symptoms and improve patient comfort. Effective communication skills are essential for educating patients and families, as well as collaborating with interdisciplinary team members to ensure cohesive care delivery. Time management and organizational skills allow the nurse to efficiently manage multiple patient visits during evening shifts while maintaining high standards of care. Proficiency with documentation and electronic medical records ensures accurate and compliant record-keeping, which is critical for continuity of care and regulatory adherence. Preferred skills such as hospice-specific certifications and experience enhance the nurse’s ability to address complex symptom management and provide empathetic support tailored to end-of-life needs.
