About the Role

As a Nurse Practitioner at Matrix, you will engage in meaningful health assessments during visits that can last up to one hour. You will review a member's health history, medication adherence, and social environment. Our Comprehensive Health Assessment (CHA) method helps us identify and close several care gaps in just one visit. Your responsibilities will include:

• Conducting thorough adult and geriatric health assessments, including medical history review, diagnosis, and treatment plans.
• Administering and ordering necessary point-of-care and lab tests.
• Collaborating with physicians, social workers, family members, and caregivers to ensure comprehensive care.
• Providing educational resources and follow-up care in conjunction with the Primary Care Physician (PCP).
• Delivering services at members' residences, whether in their homes or in Skilled Nursing Facilities.

Qualifications

• Master's Degree or equivalent experience and successful NP licensure.
• Current NP licensure in your state of practice, including prescription authority or eligibility to obtain it.
• AANP, AACN, or ANCC board certification in relevant specialties.
• Current BLS, ACLS, or CPR certification.
• At least one year of NP experience preferred; new graduates are encouraged to apply!
• A valid state driver's license and automobile insurance as required by your state.
• Strong computer skills and proficiency with medical record software.
• Excellent verbal and written communication skills.
