Registered Nurse – Case Management- Case Manager - Travel - (CM RN) StartDate: 8/25/2025 Available Shifts: 8 E Pay Rate: $2221.22 - $2447.87 Case Manager - National Care Coordination Command Center Registered Nurse, Case Manager, Discharge Planning Experience: Minimum 1 year Required Qualifications Case Manager/Utilization Review- National Care Coordination Command Center Pilot project to support in Coordination of Care for patients coming out of hospital. Plans, develops, assesses and evaluates care provided to members. Collaborates with physicians, other members of multi-disiplinary health care team and patient/family in the development. Reviews all inpatient admissions with 24 hours and begins the discharge planning process. Will work on-site at National Care Coordination Command Center; not patient facing Registered Nurse, Case Manager Experience: Minimum 1 year Licenses: RN(Compact Accepted) Work History: Should not be more than 0 day of gap Discharge Planning
