Community Support Day Coordinator

QUALIFICATIONS Minimum Education: Bachelor's degree in a behavioral health or Intellectual and Developmental Disabilities field or equivalent in experience and training. Minimum Experience: Four years' experience in the behavioral health field or Intellectual and Developmental Disabilities with at least one year supervisory experience required for staff with bachelor's degree. Two years' experience required for staff with master's degree. Qualifications for a Developmental Disabilities Professional is required. Strong organizational and communication skills required. Experience in Person-Centered Organization Skills preferred Experience in accounting/bookkeeping procedures preferred. Valid Drivers' License required. EQUIPMENT THAT REQUIRES SAFE AND EFFECTIVE USE: (Include emergency equipment, treatment equipment and high-risk equipment) Computer, printer, calculator, fax machine, telephone, copy machine, 15 passenger van, fire extinguisher, wheel chairs and other consumer adaptive equipment, ability to lift a minimum of fifty (50) pounds. All employees have a role in performance improvement and are expected to interact collaboratively with co-workers, and other contacts to provide consistent, high-quality, consumer focused services. G. AGE RELATED AND SPECIAL NEEDS OF POPULATION SERVED Work requires the knowledge and skills necessary to provide direct consumer services appropriate to Developmental Disabilities whose ages range from 18 to 65+ Staff should be able to: a. Demonstrate knowledge and skill in non-violent crisis intervention techniques. b. Display knowledge of agency, community, and institutional resources and referral processes. c. Assist consumers and staff in appropriate goal setting and implementation of the treatment plan; display knowledge of discharge planning d. Display knowledge of HFR, Regional Board, CARF, and AACSB compliance requirements. H. ASSIGNED DUTIES DESCRIPTION OF WORK DUTIES AND RESPONSIBILITIES: The agency expects employees to perform their work in a competent and efficient manner. Working as part of a team, the Community Support Day Coordinator (CSDC) will be responsible for monitoring person-centered program plans for individuals receiving day support services and living in residential settings and will train employees to properly implement the plans. Ensure the plans meet the physical, social and developmental needs of each person and there is a plan to protect each person's health, dignity, safety, comfort, well-being, civil, human and legal rights. Under limited supervision, CSDC will ensure organization and operation of services to individuals with Intellectual/Developmental Disabilities in day center and supported living settings in accordance with agency policies and procedure. The primary work location is CEM; however, per need of the service, work may occur in another Center, the residence or community. The CSDC provides direct supervision to Care Plan Coordinators and supports managers in the day-to-day oversight of instructional units and operations. INDIVIDUAL SUPPORT: Act as liaison, advocate and coordinator for individual's involvement in support groups, self-advocacy, vocational programming, transportation services, integrated community activities, recreation and learning independent living skills. Foster open and supportive communication with individuals served, families and agency stakeholders and other community agencies. Oversee individual medical, medication and health issues. Provide and ensure communication of medical and health issues to staff, agency nurses, families, etc. using appropriate logs when necessary. Implement and follow-up on all support services recommendations. Assist in the oversight of Special Olympics at local, regional and state level. SUPERVISON/TRAINING: Monitor staff performance management, including evaluations and developmental plans, disciplinary actions for all assigned Care Plan Coordinators as scheduled. Serve as Developmental Disabilities Professional (DDP) by providing observation and oversight of direct care professionals who are providing direct support services. Provide supervision to for CEM Shift Scheduling of I/DD Services. Maintain adequate staffing to ensure efficient and quality services for the maintenance of program needs. Maintain an effective direct care/support living work schedule to provide full program coverage and productivity. Reviews time and attendance records for monitoring overtime and attendance occurrence. Monitors request for leave to distribute requests for time off in fair and equitable manner. Supervise staff training and development related to Person-Centered Thinking through teaching, acting as a role model and guiding staff on how to appropriately work with and interact with individuals served, other staff, families and guardians. Work closely with agency Care Plan Coordinators to monitor day documentation for compliance including but not limited to : goal documentation, behavior tracking, progress notes, community outing logs, medication administration records. Present and implement agency policies and procedures. Participate in the interviewing and hiring of new staff. Maintain open communication with supervisor of pertinent information including but not limited to staff, scheduling, and budgeting, incident reporting. Assume SSC-2 duties in absence of supervisor. ACCOUNTABILITY: Monitors failed activities to ensure timely submission of billing claims. Ensure maintenance of licensing standards. Ensure completion/submission of all required daily, weekly, monthly, quarterly and annual reports as scheduled. Routinely review all required reports, logs, mail, requests, etc. as needed, maintaining an organized filing system. Assist in coordination of financial procedures in accordance with agency policy and procedures. OTHER: Works collaboratively with other departments to communicate medical, social and behavioral needs of individuals served in a timely manner, as well as, the daily operational needs of the program. Represent I/DD at approved community activities, health fairs and career fairs. Develop pertinent educational and trending I/DD topics for communication boards, as well as, monthly topics for Rights, Self-Preservation, Safety and Advocacy. Demonstrate and foster the characteristics of Integrity, Respect, Teamwork, Professionalism, Accountability and Innovation. Perform additional duties or special projects as required by the nature of the position or as requested by supervisor. Normal hours: Monday through Friday, 8:00 a.m.- 4:00 p.m. However, flexibility is required to work various day, evening, night and weekend shifts when needed. May be necessary to participate in emergency call rotation or cover shift if adequate coverage cannot be obtained by direct care staff. I.
