Nurse Practitioner

About the Role:
As a Nurse Practitioner, you will conduct in-depth assessments during one-hour member visits. You'll review health conditions, medical histories, medication adherence, and social risks, providing a holistic view of each member's health. Responsibilities include:
- Performing adult and geriatric assessments, including documentation, referrals, and patient education.
- Administering and ordering necessary lab tests.
- Collaborating with healthcare teams, including physicians and social workers.
- Providing services in members' residences or skilled nursing facilities.

Skills & Experience:
- Master's Degree or equivalent experience with completed NP licensure.
- Current NP licensure with prescriptive authority.
- AANP, AACN, or ANCC board certification in a relevant specialty.
- Current BLS, ACLS, or CPR certification.
- Preferred one year of NP experience; new graduates are encouraged to apply!
- Valid driver's license and automobile insurance as per state requirements.
- Strong computer skills and proficiency in medical record software.
- Excellent verbal and written communication skills.
