About the Role:

As a Nurse Practitioner, you will conduct in-depth assessments during one-hour member visits. You'll review health conditions, medical histories, medication adherence, and social risks, providing a holistic view of each member's health. Responsibilities include:

• Performing adult and geriatric assessments, including documentation, referrals, and patient education.
• Administering and ordering necessary lab tests.
• Collaborating with healthcare teams, including physicians and social workers.
• Providing services in members' residences or skilled nursing facilities.

Skills & Experience:

• Master's Degree or equivalent experience with completed NP licensure.
• Current NP licensure with prescriptive authority.
• AANP, AACN, or ANCC board certification in a relevant specialty.
• Current BLS, ACLS, or CPR certification.
• Preferred one year of NP experience; new graduates are encouraged to apply!
• Valid driver's license and automobile insurance as per state requirements.
• Strong computer skills and proficiency in medical record software.
• Excellent verbal and written communication skills.
