BASIC FUNCTION:
Under the general direction of the Executive Diretor the Bereavement Coordinator provides supportive services to help meet patient/family social and emotional needs in relation to the impact of a terminal illness, loss and grief. This includes ongoing assessments of bereavement needs, identification of ongoing individual and group support needs, direct casework, bereavement services, community education, outreach and referral.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Plans, develops, coordinates and evaluates a comprehensive bereavement program, including but not limited to mailings, phone calls, support groups, memorial services, and community outreach activities.
Provides support services to patients and/or family members.
Take an active leadership role in development of the IDT bereavement plan of care which is developed based on an assessment of the patient/family needs at the time of admission and during the provision of hospice care and when the patient dies.
Documents care provided and the response of the clients; submits documents as described in agency policy; submits written and verbal reports to Senior Management per request.
Maintains the bereavement program management system and databases.
Coordinates the activities/support of the bereavement volunteers.
Provides education to the interdisciplinary team regarding the grief process and related issues.
Provides community education and grief support services to individuals, groups, and facilities affected by loss.
Pursues and applies for grant opportunities to help support the bereavement program.
Maintain up to date records using the EMR system so problems, plans, actions and goals are accurately and clearly stated according to HGI policies, best practice and regulatory requirements.
Answer patient and families questions about medical care and the end-of-life process.
Successfully completes the annually required in-service as assigned.
Participates in QAPI program as appropriate.
Attends regular staff meetings as scheduled.
Displays critical thinking skills to problem solve and manage patient and family issues according to Hospice and Standard of Practice and HGI policies and procedures.
Informs supervisor of unusual, problematic patient/family issues.
Participates in emergency disaster processes as defined by HGI policies.
Demonstrates familiarity with HGI policies/procedures and follows state and federal regulatory requirements.
Performs assignments as directed.
Participates in Interdisciplinary team meeting as able.
Assures timely review of all emails and voice messages and responds as required.
Maintains the confidentiality of all patients.
Maintains a positive work atmosphere by demonstrating behavior and communicating in a professional manner suitable for interaction with customers, clients, co-workers and management.
Other special projects as assigned by the Executive Director
EDUCATIONAL AND EXPERIENCE REQUIRED:
Master’s in divinity, pastoral counseling, psychology, nursing, social work, or counseling. A minimum of three years' experience working within a bereavement program and or with individuals and/ or within groups regarding loss. Preferably at least two years of experience in hospice or home care. Extensive knowledge of death/dying, grief, loss and survivorship issues, clinical experience with grief support. Possess a thorough knowledge of local and community resources. The ability to work independently and within an interdisciplinary team. Possess strong communication skills in both written and verbal form
