ORGANIZATIONAL RELATIONSHIPS Position Title: Residential Care Coordinator Position Number: Date: Employee: Work Unit: Primus I/DD County: Dougherty Immediate Supervisor's Title: CEO QUALIFICATIONS (including physical requirements) Minimum Education: Bachelor's degree in a behavioral health or Intellectual and Developmental Disabilities field or equivalent in experience and training. Minimum Experience: Four years' experience in the behavioral health field or Intellectual and Developmental Disabilities with at least one-year supervisory experience required for staff with bachelor's degree. Two years' experience required for staff with master's degree. Qualifications for a Developmental Disabilities Professional is preferred. Strong organizational and communication skills required. Experience in shift scheduling preferred. Experience in accounting/bookkeeping procedures preferred. Valid Georgia Drivers' License required. EQUIPMENT THAT REQUIRES SAFE AND EFFECTIVE USE: Computer, printer, calculator, fax machine, telephone, copy machine, 15 passenger van, fire extinguisher, wheelchairs and other consumer adaptive equipment, ability to lift a minimum of fifty (50) pounds. AGE RELATED AND SPECIAL NEEDS OF POPULATION SERVED (Age related, Cultural and Linguistic Competence) Work requires the knowledge and skills necessary to provide direct consumer services appropriate to Developmental Disabilities whose ages range from 18 to 65+. Staff should be able to: Demonstrate knowledge and skill in non-violent crisis intervention techniques. Display knowledge of agency, community, and institutional resources and referral processes. Assist consumers and staff in appropriate goal setting and implementation of the treatment plan; display knowledge of discharge planning Display knowledge of HFR, Regional Board, CARF, and AACSB compliance requirements. Include % of time). Duties Include: The agency expects employees to perform their work in a competent and efficient manner. Working as part of a team, the Residential Care Coordinator will be responsible for monitoring person-centered program plans for individuals living in residential settings and will train employees to properly implement the plans. Ensure the plans meet the physical, social and developmental needs of each person and there is a plan to protect each person's health, dignity, safety, comfort, well-being, civil, human and legal rights. Under limited supervision, Residential Care Coordinator ensures organization and operation of services to individuals with Intellectual/Developmental Disabilities in group home and supported living settings in accordance with agency policies and procedures. The primary work location is PRMIUS; however, per need of the service, work may occur in the residence or community. The Residential Care Coordinator provides direct support to individuals, supervisors support staff, and supports Managers in day-to-day oversight of home operations. INDIVIDUAL SUPPORT Serve as liaison and advocate for individuals in areas such as support groups, self-advocacy, vocational programming, transportation, recreation, and independent living. Coordinate and communicate individual medical, medication, and health concerns with staff, nurses, families, and agencies using appropriate documentation systems. Implement and monitor support services recommendations to ensure follow-through. Maintain ongoing, supportive communication with individuals served, families, and community partners. SUPERVISION & TRAINING Supervise and support Residential Care Plan Coordinators, DD Nursing Staff, and the Scheduling/Administrative Assistant for I/DD Residential Services. Oversee performance management, including evaluations, development plans, and disciplinary actions. Act as DDP by conducting required ISP/HRST observations and quarterly reviews per regulatory guidelines. Ensure adequate and equitable staffing through schedule management, leave monitoring, and overtime tracking. Oversee training for assigned staff and model best practices for service delivery, professional interaction, and policy adherence. Coordinate on-call rotations and ensure schedules are available and posted in each home. Monitor residential documentation for compliance (e.g., goals, progress notes, behavior tracking, MARs, outing logs). PROGRAM COMPLIANCE & ADMINISTRATION Ensure compliance with licensing standards, agency policies, and procedures. Participate in hiring and onboarding new staff. Maintain timely communication with supervisors on staffing, scheduling, incidents, and budgeting matters. Submit and review all required reports (daily, weekly, monthly, quarterly, annually); maintain organized records and documentation. Support coordination of residential financial processes per agency policy. COLLABORATION & OTHER DUTIES Collaborate with departments to address the medical, behavioral, and social needs of individuals and support daily program operations. Uphold and model core agency values: Integrity, Respect, Teamwork, Professionalism, Accountability, and Innovation. Perform additional duties and special projects as assigned. Normal hours: Monday through Friday, 9:00 AM 5:00 PM. However, flexibility is required to work various day, evening, night, and weekend shifts when needed. May be necessary to participate in emergency call rotation or cover shift if adequate coverage cannot be obtained by direct care staff.
