Responsibilities
- Provide medical social work services to patients and families within an acute hospital setting, including assessments of psychosocial needs.
- Lead or participate in discharge planning, ensuring safe transitions and appropriate post-acute placement.
- Deliver counseling and support to patients and families; identify barriers to care and coordinate necessary resources.
- Document assessments, plans, and progress in the patient’s electronic medical record; maintain compliance with privacy and regulatory requirements.
- Collaborate with the interdisciplinary team, participate in rounds, and advocate for patient and family needs throughout the care continuum.
- Facilitate referrals to community resources, follow-up services, and care coordination after discharge as needed.

Requirements
- Active Georgia LCSW license
- Minimum of 2 years of medical social work experience in an acute hospital setting
- Acute hospital experience required
- Ability to work the listed schedule in Lawrenceville, GA
- Demonstrated ability to perform psychosocial assessments, discharge planning, and interdisciplinary collaboration
- Eligibility to work in the United States
