As a Registered Nurse specializing in case management, you will work onsite, utilizing your skills in a variety of healthcare environments across counties such as Chatham, Liberty, Bryan, and Effingham.

Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers for individualized care plans
• Address and reassess appropriate levels of care
• Implement non-medical supports for treatment plan compliance
• Coordinate care transitions and engage specialty resources
• Maintain detailed records of clinical and fiscal outcomes

Requirements of the RN - Case Management

• Unencumbered GA RN license
• Proficient with OASIS documentation
• Knowledge of HCHB and home health experience
• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license
• Case Management certification
