Registered Nurse (RN) – Home Health
Schedule: Monday–Friday, 8:00 AM–5:00 PM
On-Call Requirements: 3–4 weekend days and 5–6 weeknights per month
Productivity Benchmark: 30 points per week
License Requirement: Active Registered Nurse license in Georgia (Ringgold area)
Mileage Reimbursement: $0.43 per mile
Coverage Area: Walker, Dade, Catoosa, and Whitfield counties
Position Summary
Seeking a dedicated Registered Nurse to provide home health and case management services to individuals in community and small facility settings. This role focuses on comprehensive patient assessment, care plan implementation, disease prevention, and collaboration with interdisciplinary teams to support safe and effective care.
Primary Responsibilities
- Assess, plan, implement, and evaluate nursing care for well, chronic, and acutely ill individuals.
- Ensure health maintenance, disease prevention, and case management as appropriate.
- Work closely with healthcare professionals to plan, implement, and evaluate programs and services that address patient health needs.
- Coordinate communication and care activities across interdisciplinary teams.
- Monitor medication processes including ordering, reviewing, administering, and disposal in accordance with physician orders and policies.
- Ensure policies for medication administration, storage, errors, and destruction are in place and reviewed annually.
- Report changes in patient condition to physicians and ensure transcription of physician orders.
- Complete required daily, monthly, quarterly, and annual nursing documentation.
- Maintain accurate and routine documentation of patient health status.
- Ensure infection prevention standards are followed for furnishings, equipment, and supplies.
- Supervise and direct Licensed Practical/Vocational Nurses as needed.
- Coordinate and ensure completion of health-related appointments and timely follow-up on results.
- Communicate with inpatient nursing teams and discharge coordinators during hospitalizations.
- Provide new employee training and annual recertification on health and safety topics.
- Train unlicensed staff on individual-specific health maintenance, safety, and disease prevention needs.
Qualifications
- Active, unrestricted Registered Nurse license in Georgia.
- Home Health experience preferred.
- Experience with HCHB strongly preferred.
- Strong assessment, communication, and care coordination skills.
- Ability to supervise staff and manage complex care needs.
