Occupational Therapist (OT) – Per Diem (PRN)

Providence of Thomaston is seeking a dedicated and compassionate Occupational Therapist (OT) to join our in-house rehabilitation team on a Per Diem (PRN) basis. The Occupational Therapist will provide high-quality occupational therapy services to residents in our skilled nursing and rehabilitation setting, helping them regain independence, improve functional performance, and enhance their overall quality of life.
The ideal candidate is passionate about resident-centered care and enjoys working collaboratively with an interdisciplinary team to achieve optimal rehabilitation outcomes.
Essential Duties and Responsibilities- Conduct comprehensive occupational therapy evaluations to assess residents' functional abilities, cognitive status, activities of daily living (ADLs), and rehabilitation needs.
- Develop and implement individualized treatment plans designed to maximize independence, safety, and functional performance.
- Provide skilled occupational therapy interventions, including therapeutic activities, self-care training, cognitive retraining, upper extremity rehabilitation, adaptive equipment training, and environmental modifications.
- Evaluate and recommend adaptive equipment, assistive devices, and compensatory strategies to support resident independence.
- Monitor resident progress and modify treatment plans as appropriate to achieve established goals.
- Educate residents, families, and caregivers regarding therapy programs, home exercise programs, safety recommendations, and strategies to maintain functional gains.
- Collaborate with physicians, nursing staff, therapists, social services, and other healthcare professionals to ensure coordinated care and successful discharge planning.
- Participate in interdisciplinary care plan meetings as appropriate.
- Maintain accurate, timely, and compliant documentation in accordance with facility policies, Medicare guidelines, and state and federal regulations.
- Adhere to all professional, ethical, and regulatory standards governing occupational therapy practice.
- Master's degree or Doctorate in Occupational Therapy from an accredited program.
- Current Occupational Therapist license in the State of Georgia or eligibility for licensure.
- Current CPR certification or willingness to obtain upon hire.
- Strong clinical knowledge of occupational therapy principles, practices, and rehabilitation techniques.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary healthcare team.
- Experience in skilled nursing, subacute rehabilitation, long-term care, or post-acute care settings.
- Familiarity with Medicare documentation requirements and reimbursement guidelines.
- Experience treating geriatric and medically complex populations.
- Knowledge of adaptive equipment, environmental modifications, and functional rehabilitation strategies.
