Position Summary:

Under the leadership of the Department Director, the Licensed Practical Nurse (LPN) is an active member of the department that collects data, provides comfort measures and basic care to patients. The LPN is responsible for providing care that is appropriate for the patient’s developmental age and is culturally sensitive. The LPN follows the nursing plan of care as it has been formulated, while effectively interacting with patients, family members and other health team members.

Basic Qualifications:

Education:

Requires a high school diploma or a GED and graduation from a Licensed Practical Nurse program from an accredited vocational or community college. Requires completion of the Neonatal Resuscitation Program, Emergency Nurses Pediatric Course, and Electronic Fetal Monitoring, or equivalent courses.

Experience:

Requires up to one year of work-related experience, or any equivalent combination of education, training, and experience.

Licensure, Registrations & Certifications:

Requires a current license as a Licensed Practical Nurse and a current certification in Basic Life Saving (BLS). (If BLS verification is not current upon hire, it must be obtained within three (3) months of hire.

Essential Job Responsibilities:

• Contributes to the development of the nursing care plan through planning, data collection, appropriate implementation, and evaluation of the patient’s care, as evidenced by patient, staff and supervisor observation.

• Provides assigned patients with quality patient care services as directed by the Nurse Manager and consistent with the standards of care established by Nursing Services and CRHS.

• Functions as a part of an interdisciplinary team with positive interpersonal communication skills when communicating with patients, families, visitors, physicians and intra and inter-department staff members and representatives from other organizations.

• Documents patient care which reflects nursing intervention, patient response to care provided, and includes evidence of appropriate patient/family education, and the understanding of these instructions noted in the medical record.

• Implements and documents patient responses to treatments/medications as defined within the scope of LPN duties through the State Board of Nursing and policies of CRHS.

• Demonstrates priority setting and decisiveness that enables the completion of typical patient assignments within an assigned shift.

• Assumes responsibility for safety, infection control and performance improvement as part of CRHS’s efforts to provide quality care and services, and performs all associated duties, responsibilities, and tasks.

• Documents and reports to the medical staff observed changes in patient’s condition.

• Operates all medical equipment in a manner that ensures the patient and staff’s safety.

• Distributes all prescribed medications to assigned patients in accordance with medical staff orders.

• Complies with CRHS and department safety, infection control and performance improvement initiatives to ensure the delivery of quality care and services.

• Presents a professional image to CRHS’s nursing co-workers, clinical and medical staff members, patients, families, and visitors.

• Attends professional development programs and in-service training and meets the minimum requirement for continuing professional education.

• Performs other related job duties as assigned.

Standards of Performance:

• Performs nursing care in accordance with all established department and CRHS policies and procedures as measured by feedback and observation from nursing, medical and management staff.

• Performs all assigned patient care duties in a timely and accurate manner as measured by logs, charts, and observation.

• Complies with all safety and infection control policies and practices in the delivery of direct patient care as measured by feedback and observation from nursing, medical and management staff.

• Demonstrates reliability as measured by compliance with the CRHS Time and Attendance policy.

• Demonstrates professional conduct and adherence to all safety, confidentiality, and HIPAA standards.

• Promotes teamwork with the entire staff, as well as other contacts within the hospital, as measured by observations of manager and co-workers.

• Adheres to the CRHS Standards of Behavior as measured by observations of staff, as well as outside and other inside customers.

• Verbalizes all core competencies of the unit and department.

• Complies with all regulatory requirements including the National Patient Safety Goals as observed by peers, supervisor, and all members of the healthcare team.

• Actively participates as a member of the healthcare team by attending all mandated in-services and a minimum of 75% of the unit staff meetings and employee forums.
