About the Role:

The Hospice Social Worker in Snellville, GA plays a critical role in providing compassionate psychosocial support to patients and their families during end-of-life care. This position involves assessing the emotional, social, and spiritual needs of hospice patients, developing individualized care plans, and coordinating with interdisciplinary teams to ensure holistic care delivery. The social worker advocates for patient rights and access to community resources, helping families navigate complex healthcare systems and cope with grief and loss. By fostering open communication and providing counseling, the social worker enhances the quality of life for patients and supports families through the challenges of terminal illness. Ultimately, this role contributes to a dignified and comfortable hospice experience aligned with the values and wishes of each patient and their loved ones.

Minimum Qualifications:

  • Master’s degree in Social Work (MSW) from an accredited program.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) or equivalent in the state of Georgia.
  • Minimum of two years’ experience in hospice, palliative care, or related healthcare social work.
  • Strong knowledge of hospice regulations, psychosocial assessment techniques, and counseling methods.
  • Ability to work independently and as part of a multidisciplinary team in a fast-paced healthcare environment.

Preferred Qualifications:

  • Experience working in hospice or end-of-life care settings.
  • Certification in hospice and palliative care social work (e.g., ACHP-SW).
  • Familiarity with grief counseling and bereavement support programs.
  • Proficiency in electronic medical records (EMR) systems and documentation.
  • Bilingual skills, particularly Spanish, to better serve diverse patient populations.

Responsibilities:

  • Conduct comprehensive psychosocial assessments of hospice patients and their families to identify needs and develop appropriate care plans.
  • Provide counseling and emotional support to patients and families coping with terminal illness, grief, and bereavement.
  • Collaborate closely with interdisciplinary hospice teams including nurses, physicians, chaplains, and volunteers to coordinate patient-centered care.
  • Facilitate access to community resources, financial assistance programs, and legal support to address social determinants impacting patient well-being.
  • Document all patient interactions, assessments, and care plans accurately and timely in compliance with regulatory and organizational standards.
  • Educate patients and families about hospice services, advance directives, and end-of-life decision-making.
  • Participate in regular team meetings and case conferences to review patient progress and update care strategies.
