Registered Nurse (RN) - PRN Home Care Assessments

What You’ll Do * Perform initial and follow-up assessments for new and existing clients. * Complete 92-day reassessments and other program-required evaluations. * Develop and update individualized care plans with the assistance of Azalea admin team. * Conduct hospital discharge as needed for home care assessments * Educate clients and families on health procedures and care strategies. * Support caregiver training and in-service development. * Maintain accurate documentation to meet Georgia Medicaid / LTC Insurance / VA / Private Pay compliance standards. * Participate in occasional training sessions, meetings, or community events. What We’re Looking For * Current RN license in good standing (Georgia or Compact State). * Valid driver’s license and reliable transportation. * Current CPR/First Aid certification. * TB/Chest X-ray and fingerprint background clearance. * Excellent communication and teaching skills. * A compassionate, ethical professional who puts client care first.
