Responsibilities for a Social Worker with Phoebe Allentown:

1. Assists residents on assigned units by providing information and referral services according to residents' needs; maintains contact with area social service agencies in order to obtain information and services for residents; serves as a liaison between the facility and residents' families and/or concerned parties.
2. Performs social service assessments on residents in assigned units.
3. Sets goals for residents and documents progress as well as contact made with residents and their representatives in accordance with state regulations; prepares a discharge plan for each resident as appropriate.
4. Performs proper charting and documentation duties to ensure compliance with relevant state and federal regulations.
5. Preforms tasks which are supportive in nature to the essential functions of the position, but may be altered or redesigned depending upon individual circumstances.

Requirements for a Social Worker with Phoebe Allentown:

1. Bachelor's degree in Social Work, Social Welfare or MSW degree.
2. Working knowledge of Pennsylvania Department of Health long-term care regulations.
3. Minimum of 1 year of social work and /or related experience in assisting geriatric clients. Home & Community based service experience and dementia training preferred.
